EXPERT REPORT REQUEST AND APPLICATION FORM
Our manufacturer members need to use the following form to apply for Expert Reports required for determining production capacities, establishing raw and auxiliary material consumption rates, machinery park compliance or special regulations (Inward Processing Authorization Certificate, Waste Rates, etc.).
📝 Application Process and Points to Consider:
- Company Information: Fill in the Trade/Chamber Registry numbers, contact information and production subject fields at the top of the form completely.
- Personnel and Facility Status: Indicate the number of engineers, masters, workers and closed/open area square meters at your workplace according to current status.
- Machinery Park: Attach the list of machinery and equipment to be covered in the report to the form or enter in the relevant field.
- Approval: The completed form must be sealed and signed by the company authorized person and submitted to our Industry Directorate.
It is important that other documents requested as attachments to the form (Letter of Undertaking, Capacity Report sample, etc., if available) are also prepared for your application to be processed.
